Job summary

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health LPN to provide individualized care in the Ocala, FL area (Marion, Lake, and/or Sumter counties). The role offers flexible scheduling with 5-15+ visits per week and weekend/holiday rotations.

The LPN will deliver nursing services under RN supervision, observe patients, assist in care plans, and educate patients and families on diet, medications, and treatments.

Qualifications

• Graduate of an approved nursing program.
• Current Florida LPN license.
• Minimum 1 year experience in home health care preferred.
• IV certification preferred.

Responsibilities

• Provides nursing services in accordance with agency policy, regulations, and patient plan of care under RN supervision.
• Observes patients and reports adverse reactions to meds or treatments to Clinical Coordinator.
• Assists Physician/Registered Nurse in carrying out the plan of care and in performing procedures.
• Performs patient and family instruction on diet, medications, and treatment.

Locations: Home health visits in Ocala, FL area (Marion, Lake, and/or Sumter counties).

Schedule

PRN; 5-15+ home health visits per week. Required weekend/holiday rotations.

What You'll Do

• Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient's plan of care under the direction and supervision of a Registered Nurse.
• Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Performs patient and family instruction on diet, medication, and treatment.

What You'll need:

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one (1) year experience and/or training in home health care preferred.
• Intravenous (IV) Certification preferred.
