Empath Home Health, a member of Empath Health, seeks a caring full-time Home Health LPN to deliver individualized care in the Ocala, FL area (The Villages, Marion, Sumter, and Lake counties).

As an LPN on our team, you will provide nursing services per plan of care under RN supervision, support patients and families, and collaborate with an interdisciplinary team to ensure seamless, holistic home-care delivery.

Competenze

• Active Florida LPN license is required.
• Graduate of an approved nursing program.
• 1–2 years of LPN experience preferred.
• Home health care experience preferred.
• Intravenous (IV) certification strongly preferred.

• Provides nursing services in compliance with agency policy and patient plan of care under RN supervision.
• Observes patients and reports adverse reactions to medication or treatment.

Locations: Home health visits in Ocala, FL area (The Villages, Marion, Sumter, and Lake counties).

Schedule : M-F 40 hours; weekend and holiday rotations required.

What You'll Do

• Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient's plan of care under the direction and supervision of a Registered Nurse.
• Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Performs patient and family instruction on diet, medication, and treatment.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need
