Job summary

Empath Home Health, a member of Empath Health, seeks a compassionate PRN Home Health RN to deliver individualized care to patients in their homes across Collier County and surrounding areas, including Naples, Marco Island, Bonita Springs, Immokalee, and Ave Maria.

The role involves 10+ home health visits per week with weekend/holiday on-call rotations, collaborating with an interdisciplinary team to provide comprehensive, compassionate care and support to patients and families in their own homes.

Qualifications

• Active Florida RN license
• BSN or MSN preferred
• Minimum 1 year of direct patient care experience
• Prior home health RN experience strongly preferred
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
