Empath Home Health in the Ocala area is seeking a compassionate full-time Home Health RN to join our Home Health team. You will deliver individualized care to patients in their homes across Marion, Lake, and Sumter counties, with a 40-hour M-F schedule and on-call rotations on weekends and holidays.

Formación

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred.
• Willingness to participate in weekend and holiday on-call rotations.
• Strong assessment, critical thinking, and patient education skills.

Responsabilidades

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self‑care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Locations: Home health visits in Ocala area (Marion, Lake, and Sumter counties)
