Job summary

CareRing Health, LLC in Philadelphia is seeking an LPN to provide high-quality in-home nursing care to patients in their homes. You will collaborate with physicians, therapists, and other healthcare professionals to deliver coordinated care and ensure patient comfort and safety.

Responsibilities include conducting home visits, assessing and updating care plans, handling emergencies, administering medications, and documenting progress in the EMR system.

Qualifications

• Bilingual (Spanish) preferred.
• Active LPN state license.
• Valid driver’s license.
• Two or more years of nursing experience.

• Deliver comprehensive nursing care to patients in their homes during visits.
• Assess, plan, direct, and evaluate each patient’s care plan.
• Manage emergencies promptly and professionally.
• Complete medication administration and treatment needs for assigned patients.
• Provide nursing care in accordance with regulations and Company policy.
• Prepare and maintain accurate clinical and progress notes.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Bilingual (Spanish) preferred
• Active LPN state license
• Valid drivers' license
• Two or more years of applicable nursing experience
