Job summary

Immediate-Homecare- is seeking licensed practical nurses to provide in-home care across the North Philly area. The role involves visiting patients, coordinating with physicians, and delivering high-quality nursing services in patients' homes.

Qualifications

Responsibilities

• Deliver comprehensive nursing care in patients' homes through visits.
• Assess, plan, direct, and evaluate each patient’s care plan.
• Manage emergency situations promptly and professionally.
• Administer medications and treatments as prescribed.
• Provide nursing care in accordance with regulations and company policy.
• Prepare and maintain accurate clinical notes in a timely manner.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits


• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs


• Manage emergency situations in a prompt, precise, and professional manner


• Complete medication administration and treatment needs for all assigned patients


• Provide nursing care in accordance with applicable regulations and Company policy


• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Bilingual (Spanish) preferred


• Active LPN state license


• Valid drivers’ license


• Two or more years of applicable nursing experience
