Immediate Home Care in Philadelphia, PA, is seeking compassionate LPNs to provide top-notch home health services. This role involves conducting home visits, coordinating care among various health professionals, and managing patient care plans to meet individual needs. Candidates should hold an active LPN license and have experience in nursing, preferably bilingual in Spanish.

Formación

• Active LPN state license is required.
• Two or more years of nursing experience in a similar role.
• Bilingual (Spanish) is preferred.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Bilingual (Spanish) preferred
• Active LPN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
