Job summary

CareRing Health, LLC in Philadelphia is seeking a dedicated LPN to join our home health and hospice team. You will work with physicians, NPs, nurses, therapists, social workers, and caregivers to deliver comprehensive care in patients' homes.

The role requires assessing and updating care plans, managing emergencies, administering medications, and documenting progress. Flexible hours with full-time or part-time options, weekly pay, and comprehensive benefits are provided.

Qualifications

• Active LPN state license and current CPR certification.
• Two or more years of applicable nursing experience.
• Valid driver's license and reliable transportation.
• Bilingual Spanish preferred.

Responsibilities

• Deliver patient home visits and coordinate with physicians, NPs, nurses, therapists, social workers, and caregivers.
• Assess and update each patient’s care plan to meet needs.
• Manage emergencies promptly and professionally.
• Administer medications and treatments per policy.
• Document clinical notes and progress in a timely manner.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Bilingual (Spanish) preferred
• Active LPN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
