Qualifications

Responsibilities

• Collaborate with physicians, nurses, therapists, social workers, and caregivers to deliver in-home patient care.
• Assess and adjust each patient’s care plan to meet needs.
• Manage emergency situations promptly and professionally.
• Complete medication administration and treatment for assigned patients.
• Provide nursing care per regulations and company policy.
• Prepare accurate clinical and progress notes in a timely manner.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Bilingual (Spanish) preferred
• Active LPN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
