Job summary

Immediate Homecare and Hospice is seeking a passionate LPN to provide exceptional care to patients in the North Philadelphia area. The role involves working collaboratively with a dedicated team to assess and direct individualized patient care in their homes.

The ideal candidate will be bilingual in Spanish, hold an active LPN state license, and have at least two years of applicable nursing experience. Flexible hours and a supportive work environment will be offered.

Responsibilities

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Bilingual (Spanish) preferred
• Active LPN state license
• Valid driver’s license
• Two or more years of applicable nursing experience
