Leisure Care is seeking a Health & Wellness Nurse (LPN) to join our team and support residents in our continuing care communities. You will perform health assessments, plan services, and coordinate care with vendors to ensure high-quality living for older adults.

2+ years of assisted living or skilled nursing experience is required. Knowledge of geriatrics, dementia care, and state regulations helps you lead care on the floor and mentor staff while delivering compassionate, resident-centered

What You'll Bring:
