Job summary

Aperion Care is seeking an LPN to provide direct nursing care and to supervise nursing assistants in a facility-based setting. You will administer medications, maintain accurate documentation, and ensure patient confidentiality while following state and federal regulations.

You will collaborate with physicians and staff to coordinate care, support daily living activities, and respond to changing resident needs with professionalism and empathy. A warm, team-focused environment awaits.

Qualifications

• Licensed Practical Nurse with a current unencumbered state license.
• Experience in Long Term Care is preferred.
• CPR and First Aid certifications are required.
• Able to be flexible and adapt while handling multiple priorities.

Responsibilities

• Provide direct nursing care to residents and supervise nursing assistants.
• Administer medications as ordered by the physician.
• Complete required documentation and maintain patient confidentiality.
• Coordinate with physicians and other staff to ensure comprehensive care.
• Assist in activities of daily living and dementia care as needed.
• Report changes to clinical leadership and communicate with families.
