Job summary

Discovery Commons Orland Park is seeking a Licensed Practical Nurse (LPN) to provide compassionate, high-quality nursing care for residents in a senior living community. You will follow individualized care plans, monitor changes, and coordinate with the healthcare team to promote safety and well-being.

The position is full-time, 7am–3pm, including every other weekend, at 7420 West 159th Street, Orland Park, IL 60462, with pay rates of $36–$38 per hour.

Qualifications

• Current LPN license (or RN per state regulations) is required.
• Minimum of two years of licensed nurse experience.
• Experience in assisted living, long-term care, or skilled nursing preferred.
• Strong clinical judgment and communication skills.
• Proficiency with electronic documentation systems.

Responsibilities

• Provide nursing care per individualized care plans and policies.
• Monitor residents’ physical and psychosocial status; document changes.
• Administer medications and treatments as prescribed; document accordingly.
• Coordinate with physicians, families, pharmacies, and care providers.
• Support health promotion and wellness initiatives within the community.
• Ensure compliant clinical documentation and record-keeping.
• Contribute to care planning and resident evaluations; update service plans.
• Collaborate with caregivers and leadership to ensure high-quality care.
• Respond to emergencies with sound clinical judgment; assist staff as needed.
• Participate in training and regulatory compliance activities.
