Discovery Commons Orland Park is seeking a dedicated Licensed Practical Nurse (LPN) to join our senior living community. You will provide hands-on nursing care aligned with individualized care plans, support resident safety and wellbeing, and collaborate with an interdisciplinary team to achieve optimal outcomes.

The role requires an LPN license (or RN as per state rules), at least two years of nursing experience, and the ability to work nights and weekends.

Qualifications

• Current LPN license or RN per state regulations.
• Minimum of two years Licensed Nurse experience.
• Experience in assisted living/long-term care preferred.
• Strong clinical judgment, problem-solving and decision-making skills.
• Excellent organizational, time-management and communication abilities.
• Ability to manage multiple priorities and delegate appropriately.
• Commitment to seniors and person-centered care.
• Ability to work nights, weekends and flexible schedules.
• Proficiency with electronic documentation systems and basic computer use.
• Ability to respond to medical emergencies.
• Exposure to bloodborne pathogens and infectious disease.
• Ability to lift or assist residents as required.

Responsabilités

• Provide nursing care per resident care plans, policies, and procedures.
• Assess residents’ physical and psychosocial status; monitor changes.
• Administer medications and treatments; document accurately.
• Coordinate care with physicians, families, pharmacies, and other providers.
• Support health promotion and wellness initiatives.
• Maintain accurate, timely clinical documentation.
• Assist with care planning, service plan updates, and evaluations.
• Collaborate with caregivers, wellness team, and leadership to ensure quality care.
• Respond to concerns, incidents, and emergencies with clinical judgment.
• Support training for care staff as appropriate.
• Ensure compliance with federal, state, and local regulations.
