Job summary

Dublin Post Acute is seeking a compassionate LPN to join our Day Shift nursing team. The LPN provides direct care to residents under supervision while ensuring safety, dignity, and well-being in a post-acute and long-term care environment.

The role requires current LPN licensure in GA, CPR/BLS, and strong clinical and communication skills. New graduates welcome; supportive team and opportunities for continuing education are available.

Qualifications

• Current, unrestricted LPN license in the State of Georgia.
• Graduate of an accredited practical nursing program.
• Current CPR/BLS certification (or ability to obtain upon hire).
• Strong clinical assessment and communication skills.

Responsibilities

• Deliver quality nursing care in accordance with physician orders, care plans, and facility policies.
• Administer medications and treatments safely and accurately.
• Monitor residents' physical and emotional condition and promptly report changes to the RN or physician.
• Document assessments, treatments, medications, and resident responses accurately in the electronic medical record.
• Assist in developing and updating individualized care plans.
• Supervise and provide guidance to CNAs as assigned.
• Respond appropriately to resident needs, emergencies, and changes in condition.
• Communicate effectively with residents, families, physicians, and interdisciplinary team members.
• Maintain compliance with state, federal, and facility regulations.
• Promote infection prevention and control practices.
• Support resident rights while maintaining privacy and confidentiality.
• Participate in shift reports, staff meetings, and continuing education opportunities.
• Foster a positive, respectful, and team-oriented work environment.
