Job summary

Vitality Living St. Matthews is seeking an LPN to join our team in Louisville, KY. You will administer or assist with medication administration and treatments, coordinate with physicians, and ensure proper documentation in resident charts.

Qualifications

• Must be at least 21 years old.
• Ability to communicate effectively in English, both verbally and in writing.
• Meet state health-related requirements.
• Maintain current professional license and CPR certification if currently LPN/LVN.
• Maintain any other certifications as required by state regulations.

Responsibilities

• Administrate or assist residents to self-administer medications and treatments.
• Collect, store, and transport specimens for laboratory studies.
• Communicate with physicians via telephone, fax, and Physician Communication forms.
• Ensure physician orders, laboratory reports, and other data are scanned and filed in resident charts.
• Assist in development and revision of service plans and inform Wellness Director of resident needs.
