Job summary

Vitality Living Franklin is seeking an LPN/Charge Nurse to join our care team. This role supports medication administration, charting, and care planning for residents in a vibrant Franklin community.

The ideal candidate holds an LPN/LVN license, demonstrates strong communication, and collaborates closely with physicians and the Wellness Director to deliver high-quality resident care.

Qualifications

• Must have English communication skills, both written and verbal.
• Maintain current professional license and CPR certification if applicable.
• Meet state health requirements.
• Proficiency in coordinating with physicians and care teams.

Responsibilities

• Administer or assist residents with medications and treatments.
• Collect, store, and transport specimens for lab studies.
• Communicate with physicians via telephone, fax, and forms.
• Ensure physician orders and lab results are scanned and filed in charts.
• Assist in developing and revising resident service plans; inform Wellness Director of needs.
• Perform other duties as directed by Wellness Director.

LPN/Charge Nurse Requirements:

• Administrate or assist residents to self-administer medications and treatments
• Collect, store, and ensure transport of certain specimens for laboratory studies
• Communicate with physicians via telephone, fax, and Physician Communication forms
• Ensure physician orders, laboratory reports, and other data is scanned and filed in resident charts
• Assist in the development and revision of service plans. Inform Wellness Director of current resident needs and preferences as well as successful service approaches
• Perform other duties as directed by Wellness Director
