Job summary

CLFC Healthcare and Communications is seeking an LPN for a Lyons, NY skilled nursing facility. You will manage med passes, assist with treatments, and work closely with an RN on every shift.

The role offers a per diem, W-2, hourly schedule with 38–42 USD per hour, weekly pay, and growth opportunities, including tuition assistance up to $1,500 annually toward your RN, plus training through CLFC University.

Qualifications

• Must hold a valid New York State Licensed Practical Nurse license.
• Graduation from an approved practical nursing program.
• Current BLS (CPR) certification.
• One year of recent LPN experience.

Responsibilities

• Run the medication pass and deliver treatments per physician orders and facility policy.
• Observe, document and report changes in resident condition to the charge RN.
• Support and direct the CNAs on your unit within New York scope of practice.
• Keep shift documentation current in the facility's system.
• Follow the facility's infection control, safety and sign-in practice.
