Job summary

PureHealth is seeking an LPN to provide nursing care in a post-acute setting. You will render patient-centered care under RN direction, document in the EMR, and participate in care planning and team conferences.

Responsibilities include medication administration, infection control, and patient education. Requires LPN licensure, CPR, and strong communication; prior SNF experience is preferred.

Qualifications

• High School Diploma or Equivalent is required.
• Graduate of an accredited LPN program and valid license.
• Current CPR certification preferred or required.
• Experience in SNF or post-acute care is preferred.

Responsibilities

• Provide nursing care in assigned area according to standards and RN delegation.
• Develop individualized care plans with the team and lead conferences.
• Administer medications accurately and document in the EMR.
• Maintain communication among care team and with physicians.
• Demonstrate infection control and safety procedures.
• Educate patients and families using available materials.
