Job summary

PureHealth is seeking a licensed practical nurse (LPN) for a full-time post-acute care role. The position covers day-to-day nursing care in a post-acute setting with a 24/7 operation, including weekend and holiday shifts.

You will collaborate with Unit Manager, ARNP, and Physicians to implement care plans, administer medications, and provide patient education while maintaining compliance with policies and procedures.

Qualifications

• Graduate of an accredited LPN program.
• Valid LPN license.
• Current CPR certification.
• High School Diploma or Equivalent.

Responsibilities

• Render nursing care in assigned area per standards and nursing plan.
• Lead development of individualized care plans with Unit Manager and RN guidance.
• Administer medications accurately and monitor patient responses.
• Document observations and interventions in the EMR.
• Collaborate with physicians, nurses, and care team to adjust plans of care.
• Provide health teaching and infection control as required.
