Job summary

Clear Destination Inc. is seeking a Licensed Practical Nurse to provide nursing care to members per physician orders and emergent needs.

The role includes instruction to members, staff, and families, ensuring compliance with policies, safety, and regulatory standards. Core duties involve monitoring vital signs, documenting responses to treatments, assisting with daily activities, and delivering health education.

Qualifications

• LPN license and CPR certification are required.
• Age 21 or older.
• Preferred: 1+ year full-time equivalent healthcare experience.
• Prior experience with seniors or adults with special needs preferred.

Responsibilities

• Provide nursing care according to individualized plan and physician orders.
• Monitor and document vital signs and responses to treatments, report abnormalities to Nursing Director.
• Assist with daily activities including hygiene, meals, ambulation, and social activities.
• Deliver health education to staff, members, and family members.
• Provide emergency care as required.
• May administer medications with proper certification and supervision by RN.
• Communicate professionally with members, families, coworkers, and community.
