Job summary

PureHealth is hiring a Licensed Practical Nurse (LPN) to provide nursing care in a post-acute setting. The LPN will report to the Unit Manager and coordinate with the Registered Nurse to deliver individualized patient care in accordance with established standards.

The role requires administering medications, leading care planning, and communicating effectively with patients, families, and the health care team. It includes HIPAA compliance and ongoing professional development.

Qualifications

• Graduate of an accredited LPN program.
• Valid State Appropriate LPN license.
• Current CPR certification.
• Experience in a SNF is preferred.

Responsibilities

• Render nursing care in assigned area under RN direction.
• Develop individualized care plans with the team.
• Administer medications accurately and document in EMR.
• Provide health teaching and discharge planning information.
• Maintain effective communication with patient and family.
• Collaborate with care team to ensure quality outcomes.
• Document observations and interventions timely in EMR.
