Job summary

Rocky Mountain Care Group in Murray, UT is seeking LPNs for direct patient care in a facility setting. Positions include full-time, part-time, and PRN schedules, with compensation in the $38.00–$40.00 per hour range.

You will monitor residents, administer medications, assist with care planning, and document in EMRs while collaborating with physicians and care teams to ensure high-quality, patient-centered care.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted LPN license in practicing state.
• Current CPR certification.
• Experience in long-term care and/or home health or hospice preferred but not required.
• Food handler permit as required.

Responsibilities

• Provide direct patient care and monitor resident conditions
• Administer medications and treatments as ordered
• Monitor vital signs and report changes in condition
• Assist with assessments and care planning
• Document care accurately in electronic medical records
• Collaborate with physicians, RNs, and interdisciplinary care teams
• Provide wound care and nursing procedures within scope of practice
• Educate residents, patients, and families on care plans and treatments
• Promote a safe, compassionate, and patient-centered environment
