Rocky Mountain Care's RMCare Talent Solutions is seeking Licensed Practical Nurses (LPNs) for opportunities across our communities in Murray, Utah. Qualifications

• Active LPN license in practicing state.
• CPR certification.
• Experience in long-term care or home health and/or hospice preferred.

Responsibilities

• Provide direct patient care and monitor resident conditions
• Administer medications and treatments as ordered
• Monitor vital signs and report changes in condition
• Assist with assessments and care planning
• Document care accurately in electronic medical records
• Collaborate with physicians, RNs, and interdisciplinary care teams
• Provide wound care and nursing procedures within scope of practice
• Educate residents, patients, and families on care plans and treatments
• Promote a safe, compassionate, and patient-centered environment
