Job summary

RMCare Talent Solutions is seeking Licensed Practical Nurses to join our internal staffing team for Rocky Mountain Care. You will provide direct patient care across our care settings and coordinate with an interdisciplinary team to ensure high-quality service.

Qualifications

• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• Prior experience in long-term care and/or home health and hospice is preferred but not required.
• Food Handler Permit as required

Responsibilities

• Provide direct patient care and monitor resident conditions.
• Administer medications and treatments as ordered.
• Monitor vital signs and report changes in condition.
• Assist with assessments and care planning.
• Document care accurately in electronic medical records.
• Collaborate with physicians, RNs, and interdisciplinary care teams.
• Provide wound care and nursing procedures within scope of practice.
• Educate residents, patients, and families on care plans and treatments.
• Promote a safe, compassionate, and patient-centered environment
