ContinueCARE Hospital at Baptist Health Paducah is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct patient care in a hospital setting, ensuring comfort, safety, and well‑being, while documenting observations in the EHR.

The ideal candidate holds a current Kentucky LPN license, at least 1 year of hospital experience, and BLS certification, with strong clinical skills, sound judgment, and excellent communication.

Qualifications

• Current, active LPN license in the State of Kentucky.
• Minimum of 1 year of recent hospital experience required.
• Current BLS certification (American Heart Association or equivalent).
• Strong clinical skills and sound judgment.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced hospital environment.

Responsibilities

• Deliver direct patient care under the supervision of an RN or physician.
• Perform patient assessments and document observations and interventions accurately.
• Administer medications and treatments as prescribed.
• Monitor patient responses to treatments and report any changes to the RN or physician.
• Assist with admissions, discharges, and transfers.
• Collaborate with interdisciplinary teams to plan and implement patient care.
• Provide patient and family education and emotional support.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Uphold infection control and safety standards at all times.
