ContinueCARE Hospital at Baptist Health Paducah is seeking a dedicated Licensed Practical Nurse (LPN) to join our hospital team. The LPN will provide high-quality nursing care, support patient comfort, safety, and well-being in accordance with policies.

The successful candidate will deliver direct patient care under supervision, perform assessments, administer meds, document in EHR, and collaborate with healthcare professionals in a fast-paced setting.

Qualifications

• Active LPN license in the state of practice.
• At least 1 year of recent hospital experience.
• Current BLS certification is required.
• Strong clinical judgment and decision-making.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced hospital environment.

Responsibilities

• Deliver direct patient care under the supervision of an RN or physician.
• Perform patient assessments and document observations and interventions.
• Administer medications and treatments as prescribed.
• Monitor patient responses and report changes to the RN or physician.
• Assist with admissions, discharges, and transfers.
• Collaborate with interdisciplinary teams to plan and implement patient care.
• Provide patient and family education and emotional support.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Uphold infection control and safety standards at all times.
