Job summary

ContinueCARE Hospital at Baptist Health Paducah is hiring a Licensed Practical Nurse (LPN) to provide high-quality nursing care in a hospital setting. The position requires a current LPN license, at least 1 year of hospital experience, and BLS certification.

You will work under supervision, perform assessments, administer medications, and support patients and families. The role emphasizes compassionate, patient-centered care, accurate documentation in the EHR, and adherence to infection control

Qualifications

• Current, active LPN license in the State of Position.
• Minimum of 1 year of recent hospital experience required.
• Current BLS certification (American Heart Association or equivalent)
• Strong clinical skills and sound judgment
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast‑paced hospital environment
• Commitment to compassionate, patient‑centered care

Responsibilities

• Deliver direct patient care under the supervision of an RN or physician
• Perform patient assessments and document observations and interventions accurately
• Administer medications and treatments as prescribed
• Monitor patient responses to treatments and report any changes to the RN or physician
• Assist with admissions, discharges, and transfers
• Collaborate with interdisciplinary teams to plan and implement patient care
• Provide patient and family education and emotional support
• Maintain accurate and timely documentation in the electronic health record (EHR)
• Uphold infection control and safety standards at all times
