ContinueCARE Hospital at Baptist Health Paducah is seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a hospital setting. The LPN will support patient comfort, safety, and well‑being under supervision by an RN or physician.

The role is Full‑Time with responsibilities spanning direct patient care, assessments, med administration, documentation in the EHR, and collaboration with the care team to ensure seamless admissions, transfers, and discharge planning.

Qualifications

• Current, active LPN license in the State of Position.
• Minimum of 1 year of recent hospital experience.
• Current BLS certification (American Heart Association or equivalent).
• Strong clinical skills and sound judgment.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast‑paced hospital environment.

Responsibilities

• Deliver direct patient care under the supervision of an RN or physician.
• Perform patient assessments and document observations and interventions accurately.
• Administer medications and treatments as prescribed.
• Monitor patient responses to treatments and report any changes to the RN or physician.
• Assist with admissions, discharges, and transfers.
• Collaborate with interdisciplinary teams to plan and implement patient care.
• Provide patient and family education and emotional support.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Uphold infection control and safety standards at all times.
