Qualifications

• Current, active Licensed Practical Nurse (LPN) license in the state of Oregon.
• Ability to assess resident needs and respond appropriately to changes in condition.
• Knowledge of individualized care planning and nursing practices.
• Ability to supervise and work collaboratively with CNAs and other healthcare team members.
• Strong communication and organizational skills.
• Compassion, dependability, and a genuine desire to make a difference in the lives of others.
• Commitment to following state and federal regulations governing skilled nursing care.
• Newly licensed LPNs are welcome to apply!

Responsibilities

• Assessing resident needs and monitoring changes in condition
• Developing and implementing individualized care plans
• Providing direct nursing care and ensuring residents' needs are met
• Supervising CNAs and other care team members in the delivery of nursing services
• Maintaining accurate and timely clinical documentation
• Following applicable state and federal regulations, facility policies, and nursing standards
• Communicating effectively with residents, families, providers, and the interdisciplinary care team
• Advocating for residents and helping promote their comfort, dignity, and quality of life
