Qualifications

• Active LPN license in Oregon.

Responsibilities

• Assessing resident needs and monitoring changes in condition.
• Providing direct nursing care and ensuring residents' needs are met.
• Developing and implementing individualized care plans.
• Administering medications and treatments as ordered.
• Supervising CNAs and other care team members in nursing services.
• Maintaining accurate and timely clinical documentation.
• Communicating changes in resident condition to the care team.
• Following state/federal regulations and facility policies.
• Promoting resident comfort, dignity, safety, and quality of life.
• Collaborating with the interdisciplinary care team.
