Monarch Healthcare Management in Minnesota is seeking an LPN to provide residents with direct routine care under the supervision of the RN, aligned with Minnesota BON standards and the Resident's Bill of Rights. The LPN participates in Plan of Care development and reports changes to the RN in a timely manner.

You will collect resident health data, monitor responses to interventions, and coordinate with licensed and unlicensed staff to support residents’ well-being and care goals.

Qualificações

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• CPR certification and emergency care.

Responsabilidades

• Provide residents with direct routine care under supervision of the RN.
• Develop and modify the resident Plan of Care as delegated by RN/physician.
• Collect health data, report changes to RN, and monitor interventions.
• Assign and monitor tasks for unlicensed staff to support resident well‑being.
