Job summary

Monarch Healthcare Management in Minnesota is seeking a qualified LPN to deliver direct routine care to residents under nursing leadership. You will participate in the Plan of Care, implement delegated interventions, and monitor residents’ health status while maintaining dignity and safety.

The role requires an active Minnesota LPN license and CPR certification, focusing on teamwork, clear communication, and timely reporting of changes to the RN.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• Current CPR certification.

Responsibilities

• Provide residents with direct routine care under the supervision of nursing leadership.
• Participate in development and modification of the Plan of Care and implement delegated interventions.
• Collect resident health data and report changes to the RN.
• Assign and monitor nursing tasks to unlicensed staff based on their capabilities.
