Job summary

Aperion Care in Elgin, IL is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents and supervise nursing assistants in accordance with care plans and regulations.

You will administer medications, document care accurately, and coordinate with physicians to ensure safe, high‑quality care. Part‑time positions are available, with room to grow within Aperion Care and a commitment to supporting residents and families.

Qualifications

• LPN with a current unencumbered license is required.
• CPR and First Aid certifications are required.
• Experience in Long Term Care is preferred.
• Willing to take additional training as required by state regulations.
• Strong communication and organizational skills are essential.

Responsibilities

• Provide direct nursing care to residents and supervise nursing assistants.
• Administer medications as prescribed and ensure accurate documentation.
• Plan and oversee daily nursing activities in alignment with care plans.
• Coordinate with physicians and other staff to ensure safety and quality care.
• Receive/give nursing report during shift changes and participate in rounds with physicians.
• Assist residents with activities of daily living and provide compassionate support.
