Polaris Rehabilitation and Care Center in Cheyenne, Wyoming, is seeking a compassionate Licensed Practical Nurse to provide skilled nursing and rehabilitation care, building meaningful relationships with residents and families.

Relocation bonus available for eligible candidates; you will collaborate with nursing staff, providers, and rehabilitation professionals to support each resident’s goals, monitor conditions, and document care accurately.

Qualifications

• Active LPN license in Wyoming.
• Current CPR/BLS certification.
• Strong clinical skills, communication, and teamwork.

Responsabilités

• Monitor residents, recognize changes in condition, and report concerns to the supervising RN and providers.
• Administer medications and treatments according to orders and facility procedures.
• Participate in developing, implementing, and evaluating individualized care plans.
• Provide direction and support to nursing assistants within your scope of practice.
• Maintain accurate, timely clinical documentation.
• Communicate with residents and families about care, progress, and concerns.
• Promote resident dignity, safety, infection prevention, and quality care.
