Job summary

Polaris Rehabilitation and Care Center in Cheyenne, WY is seeking a compassionate Licensed Practical Nurse to provide skilled nursing and rehabilitation care. This role offers a relocation bonus for eligible candidates and the chance to join a dedicated care team.

As an LPN, you will monitor residents, administer treatments, contribute to care plans, and support nursing assistants while maintaining thorough documentation and strong communication with residents and families.

Qualifications

• Active LPN license authorizing practice in Wyoming.
• Current CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

• Monitor residents and report changes to the supervising RN and providers.
• Administer medications and treatments per orders and procedures.
• Develop, implement, and evaluate individualized care plans.
• Provide direction and support to nursing assistants within scope of practice.
• Maintain accurate, timely clinical documentation.
• Communicate with residents and families about care, progress, and concerns.
• Promote resident dignity, safety, infection prevention and quality care.
