Job summary

Polaris Rehabilitation and Care Center in Cheyenne, WY, is seeking a compassionate Licensed Practical Nurse to provide skilled nursing and rehabilitation care and to support resident well-being.

You will monitor residents, administer medications, contribute to care plans, educate families, and collaborate with nursing staff and rehabilitation professionals.

Qualifications

• Active LPN license in Wyoming.
• CPR/BLS certification current.
• Strong clinical skills, communication, and teamwork.

Responsibilities

• Monitor residents, recognize condition changes, and report concerns to RN or providers.
• Administer medications and treatments per orders and procedures.
• Develop, implement, and evaluate individualized care plans.
• Provide direction and support to nursing assistants within scope of practice.
• Maintain accurate, timely clinical documentation.
• Communicate with residents and families about care and progress.
• Promote resident dignity, safety, infection prevention, and quality care.
