Job summary

Polaris Rehabilitation and Care Center in Cheyenne, Wyoming, is seeking a compassionate Licensed Practical Nurse to provide skilled nursing and rehabilitation care. If you enjoy building relationships with residents and seeing their progress, this is your opportunity.

Relocation bonus available for eligible candidates. You will monitor residents, administer medications, develop and evaluate care plans, support nursing assistants, maintain accurate documentation, and communicate with residents

Qualifications

• An active Wyoming LPN license is required.
• Current CPR/BLS certification.
• Strong clinical skills, communication, and teamwork.
• Compassionate approach to resident care is essential.
• Experience in skilled nursing, rehabilitation, or long-term care is preferred.

Responsibilities

• Monitor residents, recognize changes in condition, and report concerns to the supervising RN and providers.
• Administer medications and treatments according to orders and procedures.
• Develop, implement, and evaluate individualized care plans.
• Provide direction and support to nursing assistants within scope of practice.
• Maintain accurate, timely clinical documentation.

Your role
