Job summary
Chatham Hills Subacute Care Center in Hillside, NJ is seeking a Licensed Practical Nurse (LPN) for a day shift. The role involves administering medications (including IVs), taking vital signs, and providing resident care and education.

Qualifications
- Current LPN license is required.
- Experience with medication administration, including IVs.
- Ability to take and record vital signs.
- Provide patient/family education and coordinate with care teams.
Responsibilities
- Provide nursing care for SNF and Long Term Care residents.
- Administer medications and IVs as prescribed.
- Monitor and document patient outcomes.
- Educate residents and families on care plans.
- Communicate with physicians, NPs, families and health care professionals.
- Supervise CNAs to ensure quality care.
