Chatham Hills Subacute Care Center in North Plainfield, NJ seeks a Licensed Practical Nurse (LPN) to join our day shift team and deliver high-quality care to residents in skilled and long-term units. You will administer medications including IVs, monitor vital signs, provide education to residents and families, and coordinate with physicians, NPs, and other health professionals.

You will supervise CNAs and help maintain a positive, supportive environment.

Qualifications

• Current LPN license required.

Responsibilities

• Provide care for residents in Skilled and Long Term Care Units.
• Administer medications, including IVs.
• Take and record vital signs.
• Provide resident care and monitor outcomes.
• Educate residents and families.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
