Chatham Hills Subacute Care Center is seeking a Licensed Practical Nurse (LPN) for day shift. The role covers care for residents in both the Skilled Unit (SNF) and Long Term Care Unit, including medication administration (IVs), vitals, and family education.

The successful candidate will coordinate with physicians, NPs, and the care team, and will supervise CNAs to ensure high-quality resident care. This position is offered with a competitive hourly wage and comprehensive benefits.

• Current LPN license is required.

Responsibilities

• Provide care for residents in the Skilled Unit (SNF) and Long Term Care Unit.
• Administer medications, including IVs.
• Take and record vital signs.
