Job summary

Chatham Hills Subacute Care Center in Summit, NJ is seeking a Licensed Practical Nurse (LPN) for the day shift. You will provide skilled nursing care in our SNF and long term care units, administer medications (including IVs), record vital signs, and educate residents and families. Strong communication with medical staff and supervision of CNAs are essential.

Responsibilities

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
