Chatham Hills Subacute Care Center in New Jersey is seeking a Licensed Practical Nurse (LPN) for the Day Shift to provide compassionate care in our Skilled Unit and Long Term Care Unit.

Responsibilities include administering medications (including IVs), recording vital signs, delivering care and treatments, educating residents and families, and coordinating with physicians while supervising CNAs to ensure quality outcomes.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit.
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
