Job summary

Health Advocates Network, Inc. is seeking a Licensed Practical Nurse in Lakeport, CA to deliver direct patient care under RN or physician supervision in a correctional healthcare setting.

The role supports administering medications, monitoring vitals, and implementing individualized care plans while ensuring documentation in the EMR and adherence to safety and infection control standards.

Qualifications

• Current, valid California LPN license in good standing.
• Minimum 1 year of recent LPN experience.
• Copy of Primary Source Verification (PSV) from the licensing board if required.
• Copy of all required certifications and compliance documents (BLS, TB tests, Hep B vaccination or declination, rubella/rubeola/varicella proofs).

Responsibilities

• Provide direct patient care under RN/physician supervision.
• Administer medications and treatments as prescribed.
• Monitor vital signs and document changes.
• Assist with assessments and individualized care plans.
• Perform wound care, specimen collection, and basic nursing procedures.
• Document care accurately in EMR.
• Educate patients and families on health maintenance.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean therapeutic environment.
• Follow facility infection control and regulatory standards.

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.

• Administer medications and treatments as prescribed.

• Monitor and record patients' vital signs and report changes in condition.

• Assist with patient assessments and implement individualized care plans.

• Perform wound care, specimen collection, and other basic nursing procedures.

• Document patient care accurately in the electronic medical record (EMR).

• Educate patients and families on health maintenance and treatment plans.

• Assist with admissions, discharges, and transfers.

• Maintain a safe, clean, and therapeutic environment for patients.

• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications

• Current, valid, and unrestricted CA license/certification in good standing, as required for the position.

• Minimum of 1 year of recent experience as a LPN.

• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (Healthcare Provider BLS certification; TB screening with negative PPD, QuantiFERON-TB Gold, or T-SPOT results; Hepatitis B Consent/Declination Form; and proof of Rubella, Rubeola, and Varicella vaccination or titers ).
