Job summary

Health Advocates Network, Inc. is seeking a Licensed Practical Nurse for Fond du Lac, Wisconsin. This role offers local pay at $42.16/hour and travel-related compensation opportunities for eligible candidates, including lodging and stipend components.

Qualifications

• Current, valid WI nursing license in good standing.
• Minimum of one year of LPN experience.
• A degree from an accredited school of Nursing.
• BLS certification (American Heart Association or equivalent).
• Copy of PSV from licensing board, if required.
• Copy of current required certifications and compliance documents (e.g., BLS, N95 fit test, immunization records, TB test).

Responsibilities

• Provide direct patient care under supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and record patients' vital signs and report changes in condition.
• Assist with patient assessments and implement individualized care plans.
• Perform wound care, specimen collection, and other basic nursing procedures.
• Document patient care accurately in the electronic medical record (EMR).
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, and therapeutic environment for patients.
• Follow facility policies, infection control protocols, and regulatory standards.

Local Pay: $42.16/hour

Travel Package:

Travel Rate (based on 40 hours): Hrly $18.02,Stipend (Lodging $686.00 + M&I $476.00) = $322.00

Total Comp :$1,008.00

Travel Eligibility: Candidates must live at least 50+ miles from the facility to qualify for Travel Pay.

Shift Details:

• 09:45-22:15 Every third weekend, holidays

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.

Minimum Qualifications:

• Current, valid, and unrestrictedWI license/certification in good standing, as required for the position.

• Minimum of one (1) yearof recent experience as an LPN

• A degree from an accredited school of Nursing

• BLS through American Heart Association or equivalent

• Copy of current, completedPrimary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
