Job summary

Health Advocates Network seeks a Licensed Practical Nurse (LPN) in Fond du Lac, WI to provide direct patient care under RN/physician supervision. You will administer medications, monitor vitals, and support individualized care plans in a fast-paced healthcare setting.

This role requires an active WI LPN license, minimum 1 year of LPN experience, and BLS/CPR certification. Competitive pay, benefits, and education perks are offered.

Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.

• Assist with patient assessments and implement individualized care plans.
• Perform wound care, specimen collection, and other basic nursing procedures.
• Document patient care accurately in the electronic medical record (EMR).
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, and therapeutic environment for patients.
• Follow facility policies, infection control protocols, and regulatory standards.

Local Pay: $42.16/hour - (W2)

Shift Details:

• 0545 -1815, every other weekend, Holiday PRN per unit needs.

• 36 hrs work week

Licensed Practical Nurse (LPN) Duties & Responsibilities:

Minimum Qualifications:

• Current, valid, and unrestricted WI license/certification in good standing, as required for the position.

• Minimum of one (1) year of recent experience as an LPN

• A degree from an accredited school of Nursing

• BLS through American Heart Association or equivalent

• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
