Job summary

Health Advocates Network is seeking a Licensed Practical Nurse (LPN) to join our correctional healthcare team in Canon City, Colorado. This role offers local pay of $50.00 per hour on a W2 basis and a variety of shifts.

You will provide direct patient care under supervision, administer medications, monitor vitals, and help develop individualized care plans. Eligible applicants must hold an active Colorado LPN license, have at least one year of recent LPN experience, and provide required

• Current, valid Colorado license/certification in good standing.
• Minimum of 1 year recent LPN experience (within last 3 years).
• Copy of PSV from licensing board or verification agency if required.
• Copy of required certifications (e.g., BLS, N95 fit test, immunizations, TB test).

Responsibilities

• Provide direct patient care under supervision of a RN or physician.
• Administer medications and treatments as prescribed.
• Monitor and record vital signs; report changes.
• Assist with assessments and care plan implementation.
• Perform wound care, specimen collection, and basic procedures.
• Document care in EMR accurately.
• Educate patients and families on health maintenance and plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean environment; follow policies and infection control.

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.

• Administer medications and treatments as prescribed.

• Monitor and record patients' vital signs and report changes in condition.

• Assist with patient assessments and implement individualized care plans.

• Perform wound care, specimen collection, and other basic nursing procedures.

• Document patient care accurately in the electronic medical record (EMR).

• Educate patients and families on health maintenance and treatment plans.

• Assist with admissions, discharges, and transfers.

• Maintain a safe, clean, and therapeutic environment for patients.

• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications

• Current, valid, and unrestricted CO license/certification in good standing, as required for the position.

• Minimum of 1 year/s of recent experience as a LPN within the last 3 years.

• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test,...)
