Empath Health is seeking an LPN for the Continuous Care PM shift in Venice, FL. The LPN will provide bedside clinical observations, prescribed care, and emotional support under RN supervision, covering a 1:1 patient ratio during the overnight shift (8:00 PM – 8:00 AM) with two weekend nights per month.

Mileage reimbursement is included across a coverage area including Manatee, Charlotte, DeSoto, and Sarasota counties.

Formación

• Graduate of an approved nursing program.
• Current Florida LPN license.
• Minimum one year of acute care experience; hospice LPN preferred.

Responsabilidades

• Under RN supervision, provides continuous bed-side clinical observations, prescribed care, and physical and emotional support per plan of care; reports significant changes in patient condition and family needs.
• Collaborates with RN to accept and process physician orders to maximize patient comfort.
• Assists Triage and Care Centers overnight when Continuous Care cases are pending.
