Job summary

Empath Health in Southwest Florida, as part of Tidewell Hospice, seeks an LPN to provide bedside clinical, physical, and emotional support under an RN case manager. You will report significant changes and assist families with care planning, aiming to maximize patient comfort and quality of life.

The role requires Florida LPN licensure, 1 year acute care experience (hospice preferred), and a 30-hour IV certification within 6 months of hire.

Qualifications

• Graduate of an approved nursing program with a current Florida LPN license.
• Minimum one year of acute care direct patient care; hospice LPN experience preferred.
• Completion of an approved 30-hour IV certification within 6 months of hire.

Responsibilities

• Under the RN case manager, provides bed-side clinical, physical, and emotional support as per plan of care.
• Accepts and processes physician orders with the RN to maximize patient comfort.
• Utilizes monitoring, intervention, and teaching to improve patient and family wellbeing.

Schedule: Monday - Friday 8am to 5pm

Coverage Area: Sarasota & Venice

Under the direction of RN case manager, provides clinical, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition as needed. Provides support to family/caregiver reporting psychosocial/spiritual needs to team.

What You’ll Do:

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• The LPN utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the Patient/Family unit.

What You’ll Need:

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
• Completion of an approved 30 hour IV certification course required within 6 months of hire
