Job summary

South Hills Rehabilitation Center in Eugene, OR, part of Volare Health, LLC, seeks dedicated LPNs to join a supportive nursing team. Both full-time and part-time positions are available, with competitive pay and a focus on patient comfort and quality of life.

Newly licensed LPNs welcome. You’ll assess residents, implement care plans, document care, and collaborate with an interdisciplinary team in a skilled nursing setting.

Qualifications

Current, active LPN license in Oregon.
Ability to assess resident needs and respond to changes.
Knowledge of individualized care plans and nursing practices.
Ability to supervise CNAs and collaborate with care team.
Strong communication and organizational skills.

Responsibilities

Assess resident needs and monitor condition changes.
Develop and implement individualized care plans.
Provide direct nursing care and meet resident needs.
Supervise CNAs and other team members.
Maintain accurate clinical documentation.
Follow state/federal regulations and facility policies.
Communicate with residents, families, providers, and care team.
